Out Of Network Provider
4th Trimester Physical Therapy, LLC is an out-of-network provider, which means we do not bill insurance directly. We operate on a fee-for-service basis, with payment due at the time of service. Upon request, we can provide a 'superbill' for you to submit to your insurance if you have out-of-network benefits that may cover reimbursement. We recommend contacting your insurance provider beforehand to understand your benefits.

While working with an out-of-network provider can sometimes be more costly, it comes with valuable benefits. You receive highly focused, one-on-one treatment for the entire session from a doctor of physical therapy, with a customized plan created and implemented solely by your therapist without interruptions. Most importantly, the quality of your care is not dictated by insurance limitations. You’ll also have greater communication availability between sessions, allowing for more personalized support. This dedicated and comprehensive, one-on-one approach makes a meaningful difference in both your recovery and the duration of your treatment.

Cancellation Policy :
Your appointment time is reserved exclusively for you. Late cancellations or missed appointments leave unused time that could have been offered to another patient. For this reason, we require at least 24 hours’ notice for any appointment changes or cancellations.

Patients and personal training clients who cancel with less than 24 hours’ notice will be charged a cancellation fee of 50% of the appointment cost, unless they are able to reschedule within the same week, pending availability. Patients and personal training clients who do not attend their scheduled appointment (“no-show”) will be charged 100% of the appointment cost.
